While interpersonal and intrafamilial coercion are important concerns with regard to the Bill, I am very worried about systemic and institutional forms of coercion. These operate in more subtle and nuanced ways and are often harder to detect or observe. How the option of assisted dying is framed or presented within a health or social care system can influence the choices that patients make. We see analogous patterns in other areas of healthcare. A simple example is the variations in caesarean section rates, where local culture affects whether practitioners steer patients towards one birth option over another.
